How It Works

• Book an appointment online: Choose your preferred date and time through our simple online scheduling system.
• Have the explorers complete the LiveScan Request Form: Each participant fills out the required LiveScan form before the appointment to ensure fast processing.
• ACT Biometrics provides mobile service: Our certified mobile technicians arrive on‑site with all equipment needed to complete the LiveScan fingerprinting.
• Receive your TCN after successful submission: Once fingerprints are transmitted and accepted, a Transaction Control Number (TCN) is issued for tracking and verification.

 How It Works

• Get Fingerprinted Locally: Visit any local fingerprinting provider and have  your prints captured on standard FD-258 fingerprint cards.
• Mail Your Cards: Send your completed FD-258 cards, a copy of your  government-issued photo ID, your ORI number, and payment to our Sunrise  office via secure mail (USPS, FedEx, or UPS recommended).
• We Convert & Submit: Our certified technicians digitize your fingerprint  cards using Livescan technology and submit them electronically to FDLE  and/or FBI.
• Receive Your TCN: We email your Transaction Control Number (TCN) as confirmation of successful electronic submission.

• Employer completes the requisition form: The employer must fill out the official requisition form.
• Log in to the AHCA Clearinghouse Portal: The employer initiates a new screening for the applicant inside the AHCA Clearinghouse Portal.
• Print the LiveScan Request Form:   After initiating the screening, the employer prints the LiveScan Request Form, which includes the ORI number, Screening ID, and License (OCA) number.
• Applicant books an appointment online: The applicant schedules their fingerprinting appointment through the online booking system.
• Applicant brings the LiveScan Request Form to the office and get fingerprinted.

ACT Biometrics provides fast, compliant group fingerprinting for healthcare organizations, schools, and businesses needing reliable Live Scan services for employees, volunteers, and contractors. We support large groups, onboarding events, and agency‑specific checks for FDLE, AHCA, DOH, DCF, and DBPR, coordinating on‑site sessions whether employers or employees cover the cost.

Moder or Archwell staff and managers must book and pay online for fingerprint card conversion. The date selected does not matter—the applicant does not need to visit our office. Booking is for payment only.

Special Rate: $150 (regularly $175)
