✅ Step 4.1: Create or Update Your ProfileGo to: https://chai.flclearinghouse.com
- Enter your email address to receive a one-time passcode.
- Important: Keep the CHAI page open while retrieving the code from your email.
✅ Step 4.2: Create or Update Your Profile
- If no profile is found, click “Initiate Screening.”
- Enter your personal information, including your ITIN in place of SSN.
- Provide your date of birth, address, and other required details.
✅ Step 4.3: Enter Prior States of Residence
- List any U.S. states you’ve lived in during the past 5 years.
✅ Step 4.4: Select Your Provider/Position
- Choose the appropriate provider or licensing board (e.g., Florida Board of Nursing).
- Select your position type (e.g., RN, LPN, CNA).
✅ Step 4.5: Acknowledge the Privacy Policy
- Scroll to the bottom of the privacy policy window and check the acknowledgment box.
- On mobile devices, rotate your screen to landscape mode if the checkbox is not visible.
✅ Step 4.6: Skip Appointment Booking
- When prompted to select a fingerprint vendor, scroll down and click:
“Continue without Making an Appointment.”
- This will generate your Livescan Request Form.
✅ Step 4.7: Download & Print the Livescan Request Form
- The form will include your Screening ID and ORI number.
- Print the form or take a screenshot—you’ll need it for fingerprinting.
