Instructions for MedPro International Applicants

1. Schedule a Card Conversion Service
2. Apply for an ITIN (Individual Taxpayer Identification Number) or Complete the Fingerprinting Request Form to Submit Without a Social Security Number or ITIN Number
3. Complete Fingerprinting
4. Register with the CHAI Clearinghouse (if applicable)
5. Mail Required Documents (Fingerprint Cards and a copy of the applicant's Passport to ACT Biometrics.
6. Email a passport-style photo (selfie) taken against a white background to livescan@actbiometrics.com. The photo must meet the following requirements:

• No glasses
• No hat or head covering
• Do not smile
• Must be in PNG or JPG format (No PDF)

MedPro can make an individual payment for the service using the link below. Alternatively, we will send an invoice directly to MedPro for processing.  

Florida requires all screened‑profession background checks to go through the Clearinghouse, and applicants without a Social Security number must provide an ITIN. Certain applicants may complete screening without an SSN, but their results must be manually processed. A valid Social Security number is still required before a license can be issued. Applicants who need to apply for an ITIN instead of submitting fingerprints for licensing without a Social Security number may use the link below to complete the ITIN application. 

• Complete 1–2 FD-258 fingerprint cards.
• These can be done at a local police station or fingerprinting agency.

✅ Step 4.1: Create or Update Your ProfileGo to: https://chai.flclearinghouse.com

• Enter your email address to receive a one-time passcode.
• Important: Keep the CHAI page open while retrieving the code from your email.

✅ Step 4.2: Create or Update Your Profile

1.   If no profile is found, select “Initiate Screening” and then click “No Social Security Number (SSN) or ITIN?”
2. Enter your personal information, including your date of birth, place of birth, mailing address, telephone number, email address, and any other required details.
3. Select your Identification Type (I‑9, Passport, or SEVIS).
4. If you choose “Passport,” enter the 9‑digit passport number and upload a copy of the first page of your passport.

• List any U.S. states you’ve lived in during the past 5 years.

• Choose the appropriate provider or licensing board (e.g., Florida Board of Nursing).
• Select your position type (e.g., RN, LPN, CNA).

• Scroll to the bottom of the privacy policy window and check the acknowledgment box.
• On mobile devices, rotate your screen to landscape mode if the checkbox is not visible.

• When prompted to select a fingerprint vendor, scroll down and click:
  “Continue without Making an Appointment.”
• This will generate your Livescan Request Form.

• The form will include your Screening ID and ORI number.
• Print the form or take a screenshot—you’ll need it for fingerprinting.

Mail the following to ACT Biometrics:

1. 1–2 completed FD-258 fingerprint cards
2. Printed Livescan Request Form
3. Copy of ITIN
4. Copy of a valid photo ID (passport, driver’s license, or state ID)
5. Passport-style photo (white background)
