ACT Biometrics Card Conversion Services

At ACT Biometrics, we understand that not everyone can travel to Florida to complete their fingerprinting requirements. That's why we offer convenient card conversion services for applicants living overseas or out-of-state, especially those who need to submit their background check results to the Agency for Healthcare Administration (AHCA).

How It Works:

1. Book an Appointment: Schedule your card conversion service online and provide some basic information.
2. Fingerprinting: Have your fingerprints taken on an FD-258 fingerprint card at your local police station or sheriff’s office. Ensure the card is filled out completely and signed by the official taking your fingerprints. For your convenience, we can mail you the FD-258 fingerprint card for a $25 fee and virtually assist you with taking your fingerprints before you mail the cards back to us. Alternatively, you can download the FD-258 sheet online from the FBI website.
3. Mail Required Documents: Send the following to our address:

• One or two fingerprint cards
• A legible photocopy of your government-issued photo ID (passport or driver’s license)
• A cashier's check or money order if you choose not to book and pay online

Additional Requirements for Healthcare Professionals:

If you are applying for a healthcare professional license or submitting results to AHCA, please email a photograph taken against a neutral colored, plain background to livescan@actbiometrics.com. The photograph should be similar to a passport photo, with a neutral facial expression and no glasses, caps, or facial covering. Ensure the photo is sent as a .JPG attachment.

Electronic Submission:

Once we receive your fingerprint card(s), we will convert them into livescan and electronically submit the fingerprints to law enforcement. They will then send the background check results to the appropriate requesting agency based on the ORI number provided. You can obtain the ORI number from your employer or the agency requiring the background screening. This information can usually be found on their website or by contacting them directly.

Payment Information:

Applicants can pay the service fees online by clicking on "Book" below. Please note that we cannot accept International Money Orders. If you live outside of the United States, please make your payment with a major credit card.
