steps to register with ahca clearinghouse

1. Visit the CHAI Clearinghouse website.
2. Enter your email address to begin.
3. You’ll receive a confirmation code via email.
4. Use that code to log in and enter your Social Security  Number or  TIN, date of birth, and last name.
5. Skip the appointment booking section—this step is not needed for card conversion.
6. Download the Livescan Request Form, which includes your Screening ID and ORI number.

1. Visit your nearest police station and request 1–2 fingerprint cards (FD-258 format preferred).
2. Download the FD-258 template from their website if needed.
3. Mail the following items to ACT Biometrics: 
   • ✅ Completed fingerprint cards
   • ✅ The AHCA Livescan Request Form

           4. Email a passport-style photo (required for AHCA submission).
